
Requirements 
General Requirements
- Online application form (Click here)
- Visa form is to be filled out online.
- Application forms filled out manually will NOT be accepted.
- Valid Passport
- Passport valid for 90 days after the applicant’s last day of stay in the Schengen territory with two blank visa pages, issued within last 10 years plus the copy of the passport (page with data and photograph)
- Photo
- One recent colour good quality passport size photograph, with a white background in colour
- Ticket and Itinerary
- Round trip air ticket and itinerary (itinerary when applying for the visa, ticket when the visa is collected.)
- Financial means
- Proof of financial funds for the stay at the territory of Schengen States (stamped bank account/credit card statements over the last three months)
- Cash will not be accepted as proof of foreign exchange.
- Travel insurance
- Proof of travel/health/ insurance for the entire duration of the stay in the Schengen territory (schedule or benefits) with name, policy number, the period of cover and medical coverage of Rand equivalent of 30 000 Euro.
- Accommodation
- Proof of accommodation (hotel reservation/invitation letter from a private person issued by Wojewoda (District Authority Office in Poland)
- Proof of Employment
- The letter must state the employee’s name, their position, their length of service and nature of service (permanent/contractual work).
- The letter must indicate that the employer is aware that the employee will be taking leave on the said dates and that the employee will resume his/her duties upon return.
- The letter must be written on company letterhead and must have the date of issue, address, telephone number and position of the signatory.
- The letter also needs to state the reason for travel
- Self-employed should present a registration document of the business with the relevant South African authority. 1) CIPC, Health Professions Council of South Africa or other applicable authority. 2) Registration with the South African Revenue Service (SARS). 3) If applicable, a SAICA accredited chartered accountant’s letter stating the current financial standing of the business.
- Proof of studies letter for students. Students must present a recent, official and signed letter from the school, college or university, stating the date of issue, the name of the applicant and the type of studies or the name of the courses the student is enrolled in according to the university’s/school’s records
Additional requirements for Business visas
For business, an official letter from a company/institution in Poland and from the employer of the applicant.
- Invitation letter
- Invitation letter from the company or business partner in Poland/in the Schengen Member State territory.
- If the trip includes visits to several countries within the Schengen Member State territory, all countries to be visited must be mentioned in the invitation letter.
- NB: The invitation letter should motivate the need for a multiple entry Schengen visas if it is foreseeable that the applicant will have more business visits to the Schengen Member State territory.
Additional Requirements for Minors
- Certified parental consent from both parents for children under 18 traveling alone.
- If the child travels with one parent, the consent of the other parent must be produced, if only one parent has the guardianship of the minor, the certified court document stating so must be presented.
- The unabridged birth certificate must be submitted to identify the parents of the child.

Important note
- Applicant does not have to do biometrics if he had a previous Schengen Visa issued within 59 months with the remark “VIS” in the Visa (Sample of VIS)
- Where the Issuing Authority requires applicants to present themselves for biometrics ON SUBMISSION. - Visas are required by South African passport holders.
- Visa applications will be rejected if application forms are not fully completed & signed & are not accompanied by the required documents.
- The possession of a visa is only one of the pre-requisites for entry into the territory of the Schengen state’s Border, authorities may refuse entry, no claim to reimbursement of the visa can be submitted.
- The Schengen state’s consular offices reserve the right to request further documentation should it be deemed necessary.
- For organized groups, arrangements should be made well in advance with the relevant issuing authority.
- There is no guarantee that a visa will be issued.
- Visa processing fees are not reimbursable if a visa is rejected.
